IN 2012 AND 2013,
Langlade
Hospital, along with numerous
community partners, conducted a
Community Health Needs Assessment to
determine the troubling health problems
facing the people living in Langlade
County and surrounding communities.
The long-term goal is to improve the
health of people in our community.
The following health initiatives were
determined, and action plans were
established to positively affect these
priority areas:
w
Affordability of health care and
prescription medication costs.
w
Obesity.
w
Wellness and prevention services.
w
Substance abuse and chemical
dependency.
Coming together to meet
these goals
In an effort to support achievement
of the goals established, the Langlade
County Health Coalition was created.
It includes community representatives
from health organizations, social service
agencies, schools and law enforcement,
as well as county and local government
and other local partners.
Four teams were created within the
coalition to address each of the health
initiatives. Team members are meeting
monthly to help identify barriers within
each initiative and to develop plans to
address the problems.
Receiving specialized training
The Langlade County Health Coalition
was selected to participate in the Healthy
Wisconsin Leadership Institute’s
Community Teams Program (a joint
program of the UW School of Medicine
and Public Health and the Medical College
of Wisconsin). This is a year-long leadership
training program that helps prepare
participants to address their communities’
most pressing health concerns.
The team will receive training and
technical assistance in:
w
Building stronger community
partnerships.
w
Implementing sustainable change
strategies.
w
Measuring their own success from
national and state experts.
In addition, Langlade Hospital
recently appointed a Community Health
Improvement Specialist to work with the
Coalition to assist with the development
and implementation of the strategies
determined.
